Pop Warner Registration Requirements

OPRA Sports Pop Warner Football requirements for the 2010 season are now available.  All items are required!

1. COPY of certified birth certificate or current passport (will not be returned)
2. OP Football Registration form.   
3. Pop Warner Player Contract.
4. Copy of 2009/2010 Full Year Report Card
5. 2010 Pop Warner Physical Form.
6. Registration Fee $260 (must add family fee of $75 if not already paid in 2010).
7. Equipment Deposit check of $125 also required (will not be cashed). Checks to OP Football

Don't forget to schedule your physical with your physician before July 18 so that all forms will be complete on time. Be sure to take the Pop Warner Physical form (no other form will be accepted).

In person registration will beging April 17, 2010 at the JAMBOREE.  The registration process is changing this year.  You will have to fill out the OP Football form and pay your registration fee.  All other forms will be collected on the date your child receives his equipment.   If you have played before, you will remain with the same players as before.  If you wish to play with another coach or have not played before, there will be an evaluation on July 14, 2010 at South Charlotte Middle School.
